Safety Orientation Acknowledgement Form
The Allfirst safety orientation program for new employees or contract employees is administered on a pre-employment basis. Each potential new employee or contract employee, as a pre-condition to employment will be orientated on the following safety issues by video or by professional instructor. After instruction on these topics, a test will be given and there must be a satisfactory performance level for employment consideration.
I hereby acknowledge that I received training by video or by professional instructor on the below topics and received my safety equipment. I fully understand all of the expectations and/or requirements outlined in the program. I will contact the Safety Director with any questions prior to starting working activities.
